Background This study assessed patient perceptions of the Physician Payments Sunshine Act and opinions toward physicians who receive gifts and/or payments from pharmaceutical or medical device companies. Methods During their office visit, patients attending different specialty clinics volunteered to complete our survey. The survey asks if the patient knows what the Sunshine Act is, then asks questions on 5-point response scales to assess the patient's opinions toward physicians who receive compensation from companies, their self-rated knowledge of physician compensation, and how (...) they believe this compensation affects the cost of care. Results Over 13 months, 523 responses were collected: 8.6% of patients reported having knowledge of the Sunshine Act, 56.8% rated their knowledge of physician compensations as “poor,” and 67.1% agreed with the statement that patients should be aware of the compensation physicians receive. When asked how their opinion toward their physician would change if they learned the physician received free meals or gifts from companies, 58.9% replied “not at all,” and 36.11% of patients did not believe their cost of care would increase if their physician received compensation from companies. Conclusions Most patients were unfamiliar with the Sunshine Act, and believe their knowledge of physician compensation is poor. Over half of the respondents would not change their opinion of their physician based on knowledge of their physician receiving payments/gifts from companies, and over one-third of respondents did not believe such compensation increased the cost of care. The majority of respondents agreed that patients should be aware of payments/gifts to physicians. (shrink)

What drives organisations to engage in socially responsible purchasing initiatives? To investigate this important question, this article uses a case-study approach to examine the context within which supplier diversity programmes have emerged in both the U.S. and U.K. The analysis identifies legislative and policy developments, economic imperatives, stakeholder pressures and ethical influences as forces shaping organisational responses. It reveals important contextual differences between U.K. and U.S. experience and offers an empirical and theoretical explanation of corporate behaviour.

Rebb Binyamin (pseudonym of Yehoshua Radler-Feldman; 1880–1957) was a leading figure in movements that called for the establishment of a joint Jewish-Arab political framework in Palestine and that sharply criticized the Zionist cooperation with the British colonial authorities. In the early 1920s, he began exploring the writings of Mohandas K. Gandhi (1869–1948) as the basis for his critical approach toward the hegemonic Zionist discourse. In his writings Rebb Binyamin emphasized Gandhi’s refusal to reconcile himself to the British colonial “divide and (...) rule” policies by creating divisions between Hindus and Muslims, the Indian anticolonial struggle that he led, and the way this struggle inspired the development of anticolonial movements in the Middle East. These aspects of Gandhi’s political repertoire were considered by Rebb Binyamin as a framework for the critical discussion of the ways in which the British colonial authorities perceived the Zionist movement as their representative and pointed to the fact that the Zionist-colonial partnership was an obstacle to the crystallization of Jewish-Arab cooperation. This article focuses on the ways in which Rebb Binyamin’s reading of Gandhi served as the basis for his critique of hegemonic Zionist discourse and practices. (shrink)

There are many possible approaches to describing the effects and uses of music in a particular society. It would be a mistake to assume that music in the Bible is not the cement of social life and has no liturgical significance. The present study seeks to explore how people in ancient times employed music using the harp and the ram's horn , to cope with roles that were open or never-ending in their demands. In particular, it focuses upon the role (...) played by music in secular life as well as religious rituals, as described in the Bible. The method used was an extensive literature study of the Old and New Testament, textbooks and relevant peer reviewed journals, with a focus on both secular and religious reasons for singing, dancing and playing instruments. It was found that the Bible described the use of the harp and the ram's horn associated with singing and dancing during celebrations, for military use, as well as mourning or lamentation. It was concluded that music using different instruments, including the harp and shofar, helped people to cope with the demands of everyday life and thus played an important role in both secular life and religious rituals in biblical times. Languages and writing systems result from satisfying multiple constraints related to learning, comprehension, production, and their biological bases. Orthographies are not optimal because these constraints often conflict, with further deviations due to accidents of history and geography. Things tend to even out because writing systems and the languages they represent exhibit systematic trade-offs between orthographic depth and morphological complexity.

In matters of discontinuation of life-sustaining treatment, traditional role of the family to speak on behalf of the incompetent patient is questionable. We explore the reasons why physicians perceive patient autonomy to be transferrable to family members. Principle of patient autonomy may not suffice when futile treatment is demanded and may serve to erode the ethical integrity of medical profession. An enhanced role for bioethics committees is proposed when physicians propose to discontinue life-sustaining treatment against the wishes of the patient (...) or their families. (shrink)

Although deductive reasoning is a closed system, one's beliefs about the world can influence validity judgements. To understand the associated functional neuroanatomy of this belief-bias we studied 14 volunteers using event-related fMRI, as they performed reasoning tasks under neutral, facilitatory and inhibitory belief conditions. We found evidence for the engagement of a left temporal lobe system during belief-based reasoning and a bilateral parietal lobe system during belief-neutral reasoning. Activation of right lateral prefrontal cortex was evident when subjects inhibited a prepotent (...) response associated with belief-bias and correctly completed a logical task, a finding consistent with its putative role in cognitive monitoring. By contrast, when logical reasoning was overcome by belief-bias, there was engagement of ventral medial prefrontal cortex, a region implicated in affective processing. This latter involvement suggests that belief-bias effects in reasoning may be mediated through an influence of emotional processes on reasoning. (shrink)

In a companion review1 we discussed the data supporting the conclusion that at least two subtypes of spectrin exist in mammalian brain. One form is found in the cell bodies, dendrites, and post‐synaptic terminals of neurons (brain spectrin(240/235E)) and the other subtype is located in the axons and presynaptic terminals (brain spectrin(240/235)). Our recent understanding of brain spectrin subtype localization suggests a possible explanation for a conundrum concerning brain 4.1 localization. Amelin, an immunoreactive analogue of red blood cell (rbc) cytoskeletal (...) protein 4.1, is localized in neuronal cell bodies and dendrites when brain sections are stained with antibody against rbc protein 4.1. However, it has recently been suggested that synapsin I, a neuron‐specific phosphoprotein associated with the cytoplasmic surface of small synaptic vesicles, is related to erythrocyte 4.1. In this review we hypothesize that there are at least two forms of brain 4.1: a cell body/dendritic form (amelin) which is detected with rbc protein 4.1 antibody, and a unique form found exclusively in the presynaptic terminal (synapsin I). The binding of synapsin I to brain spectrin(240/235), and its ability to stimulate the spectrin/F‐actin interaction in a phosphorylation‐dependent manner suggests a model for the regulation of synaptic transmission mediated by the neuronal cytoskeleton. Cognitive science uses the notion of computational information processing to explain cognitive information processing. Some philosophers have argued that anything can be described as doing computational information processing; if so, it is a vacuous notion for explanatory purposes.An attempt is made to explicate the notions of cognitive information processing and computational information processing and to specify the relationship between them. It is demonstrated that the resulting notion of computational information processing can only be realized in a restrictive class of dynamical (...) systems called physical notational systems (after Goodman's theory of notationality), and that the systems generally appealed to by cognitive science-physical symbol systems-are indeed such systems. Furthermore, it turns out that other alternative conceptions of computational information processing, Fodor's (1975) Language of Thought and Cummins' (1989) Interpretational Semantics appeal to substantially the same restrictive class of systems. The goal of this article is to trace the transformations in Nietzsche's early thinking that led to the ideas published in Human, All Too Human, the first book of his mature philosophy. In contrast to his early works, in which he sides with art and philosophy in criticizing the scientific culture of his time, Nietzsche, in Human, All Too Human, hails the methodology of science as a way to overcome the metaphysical delusions of philosophy, art, and religion. However, in disagreement (...) with popular scholarly view, I show that Human, All Too Human does not represent a complete break with his earlier writings, but rather a shift in emphasis. I argue that the overall project connecting Nietzsche's early thought to Human, All Too Human is, first, the attempt to come to terms with the upheavals introduced into the cultural terrain by the Socratic turn, and second, to affirm a higher culture. We address Northcott’s criticism of Pearson’s correlation coefficient ‘r’ in measuring causal strength by replacing Pearson’s linear regressions by nonparametric nonlinear kernel regressions. Although new proof shows that Suppes’ intuitive causality condition is neither necessary nor sufficient, we resurrect Suppes’ probabilistic causality theory by using nonlinear tools. We use asymmetric generalized partial correlation coefficients from Vinod [2014] as our third criterion in addition to two more criteria. We aggregate the three criteria into one unanimity index, UI in [-100; 100], (...) quantifying causal strengths associated with causal paths: Xi -> Xj, Xj -> Xi, and Xi Xj. (shrink)

This volume explores Nietzsche's decisive encounter with the ancient philosopher, Epicurus. The collected essays examine many previously unexplored and underappreciated convergences, and investigate how essential Epicurus was to Nietzsche's philosophical project through two interrelated overarching themes: nature and ethics. This paper argues that for Nietzsche it is only when the strong type decays on its own terms that it is possible for a weak type to come into dominance by inverting the values of the strong. It sets right a latent inconsistency in Deleuze’s work, Nietzsche and Philosophy , which traces back the origin of decadence to the subterranean struggle between reactive forces. I show that Deleuze’s reading runs contrary to his own contention that for Nietzsche the negative is (...) a result of activity. I argue that the decadence of the strong is a consequence of its very creative activity and abundant health. Relying on Nietzsche’s later works, I isolate and analyze four typical traits of the strong type which not only determine this type as what it is but also make it vulnerable to decadence. (shrink)

In this paper I argue that Direct realism is less prone to internal incoherence as a theory of knowledge than alternative theories. The theory of Direct realism best grounds our capacity for cognition of the external world, whereas other epistemological theories claim to ground our capacity for cognition, but end in skepticism in the final analysis. I propose to show this contrast by bringing in different theories of a well-known Indirect realist, Bertrand Russell. Illustrating this point mainly through Russell makes (...) the approach purely historical in nature even when the paper includes a brief discussion of two theories in the current debate that support Direct realism. The unknown nature of tomorrow’s research makes informed consent in biobank research a challenge. Whether the consent given by biobank participants is ‘broad’ or ‘narrow’, the ever present question remains the same: are new activities covered by the original consent? In this article, we focus on the meaning of, and the relation between, broad consent and re-consent in biobank research. We argue that broad consent should be understood as consenting to a framework—a framework which covers aims, core conditions for acceptable (...) use, governance and how these affect participants. Changes that alter the framework in a fundamental way call for re-consent. Three biobank cases of current international interest are used to debate when re-consent is an ethical necessity: whole-genome sequencing, data sharing and commercial utilization. These reflections give us a more nuanced view on what consent is for. We claim that the introduction of broad consents in biobank research has not represented a betrayal of individual participant interests, as some critics have asserted. Broad consents combined with the possible use of re-consent are in certain settings not inferior, but rather ethically superior to narrow consents. In population-based research biobanks, they allow for a reconciliation between individual interest and public matters in society at large. (shrink)

Increasingly, national governments across the globe are prioritizing investments in neuroscience. Currently, seven active or in-development national-level brain research initiatives exist, spanning four continents. Engaging with the underlying values and ethical concerns that drive brain research across cultural and continental divides is critical to future research. Culture influences what kinds of science are supported and where science can be conducted through ethical frameworks and evaluations of risk. Neuroscientists and philosophers alike have found themselves together encountering perennial questions; these questions are (...) engaged by the field of neuroethics, related to the nature of understanding the self and identity, the existence and meaning of free will, defining the role of reason in human behavior, and more. With this Perspective article, we aim to prioritize and advance to the foreground a list of neuroethics questions for neuroscientists operating in the context of these international brain initiatives. Paul Raimond Daniels’s Nietzsche and The Birth of Tragedy is an engaging, instructive, and clearly written study of Nietzsche’s first book. It is a particularly fine achievement given the difficulties, in terms of both style and content, that Nietzsche’s text presents to the reader. Daniels’s aim is to present BT as an ideal introduction to Nietzsche’s philosophy, and, in light of its problematizing of the relation between art and truth, to argue that BT is crucial for evaluating the aims, successes, (...) and shortcomings of Nietzsche’s later philosophy. Furthermore, Daniels presents an “affirmative” interpretation, arguing that BT champions a life-affirming worldview that finds its highest expression in... (shrink)

Two recent works will be considered that discuss Heidegger and Nietzsche in the context of the problem of overcoming metaphysics and the onto-theological tradition. A common criticism will be that these works in their attempt to retrieve a conception of religion, politics or faith beyond onto-theology and metaphysics tend to justify or idealize particular historically and culturally conditioned perspectives, which are not immune from further philosophical critique. Simon Oliai’s approach to countering the burgeoning fundamentalisms in our global age is ultimately (...) not very convincing, representing as it does, a Western, liberal, Eurocentric worldview. The author’s use of Nietzsche or Heidegger for this purpose is also not very compelling. The remainder of this review essay appraises the volume edited by Cimino and van der Heiden, which attempts to rethink faith beyond onto-theology in several interesting ways. Although meant to be a comparative study, several contributions subscribe to a general Heideggerian (or Heidegger-inspired phenomenological-hermeneutical) theoretical framework, which itself is not rigorously scrutinized. A problematic implication is that Nietzsche’s own critical and constructive views of philosophy, his critique of religion and history, and his positive appraisal of faith and divinity are not seriously considered on their own terms: they are either mostly ignored, or they are selectively interpreted through a Heideggerian lens, with the result that this volume puts forward a mostly unfavorable view of Nietzsche. Some of the conceptual moves credited to Heidegger in the volume have their equivalents in Nietzsche’s philosophy, which a comparative and evaluative study such as this one must seriously take into account by resisting the uncritical acceptance of Heidegger’s own view of Nietzsche. These are simple expressions of a searching mind trying to understand itself and Nature. Such a mind is amazed by the wonder of the humanbeing and the intricate inter-relatedness in Nature. In its wanderings, it stumbles upon the unique peace in itself and is struck by the glory of this amazing experience. A sense of joy (...) and creative happiness dawns on such a mind. The book is an attempt to express this unique wonder of "growing out of oneself" and realizing humanity as truly an integral part of this incredible natural process. We respond to Farah (1994) by making some general remarks about information encapsulation and locality and asking how these are violated in her computational models. Our point is not that we disagree, but rather that Farah's treatment of the issues is not sufficiently rigorous to allow an evaluation of her claims.

Increasing university students’ engagement with ethics is becoming a prominent call to action for higher education institutions, particularly professional schools like business and engineering. This paper provides an examination of student attitudes regarding ethics and their perceptions of ethics coverage in the curriculum at one institution. A particular focus is the comparison between results in the business college, which has incorporated ethics in the curriculum and has been involved in ethics education for a longer period, with the engineering college, which (...) is in the nascent stages of developing ethics education in its courses. Results show that student attitudes and perceptions are related to the curriculum. In addition, results indicate that it might be useful for engineering faculty to use business faculty as resources in the development of their ethics curricula. (shrink)

In the last decade, reading research has seen a paradigmatic shift. A new wave of computational models of orthographic processing that offer various forms of noisy position or context-sensitive coding have revolutionized the field of visual word recognition. The influx of such models stems mainly from consistent findings, coming mostly from European languages, regarding an apparent insensitivity of skilled readers to letter order. Underlying the current revolution is the theoretical assumption that the insensitivity of readers to letter order reflects the (...) special way in which the human brain encodes the position of letters in printed words. The present article discusses the theoretical shortcomings and misconceptions of this approach to visual word recognition. A systematic review of data obtained from a variety of languages demonstrates that letter-order insensitivity is neither a general property of the cognitive system nor a property of the brain in encoding letters. Rather, it is a variant and idiosyncratic characteristic of some languages, mostly European, reflecting a strategy of optimizing encoding resources, given the specific structure of words. Since the main goal of reading research is to develop theories that describe the fundamental and invariant phenomena of reading across orthographies, an alternative approach to model visual word recognition is offered. The dimensions of a possible universal model of reading, which outlines the common cognitive operations involved in orthographic processing in all writing systems, are discussed. (shrink)
